By June 2018 there will been improvement in the accuracy of patient information transfer between the Charge Nurse in the Emergency Department and the Patient Care Coordinator (PCC) at UHNBC, through the use of a standardized communication tool.
A brainstorming session between PCCs and Charge Nurses was held to determine critical pieces of information about the patients that needed to be transferred from one to the other. This information was used to inform changes to the their communication tool. The changes to the communication tool were tested with 6 revisions until the tool was considered more effective and useful.
