All patients that present to the Hudson’s Hope Health Centre will have a CTAS, demographic verification, triage and seen by the clinicians in an appropriate manner. We will achieve 100% compliance within 6 months.
Data Collection and process mapping discovered the errors in data collection. Also it was noted the wrong professional was seeing the patient at the wrong time. New process was created and agreed upon. Ongoing monitoring of the data.
