The Chronic Disease Community of Practice (CoP) Steering Committee invites you to this webinar to review the direction of the CoP and proposed focus groups, based on feedback and themes from the April 2023 CoP Engagement Event. The CoP aims to build on innovations and leadership in the province to improve the care of patients at risk for, or living with, chronic disease(s).

Half of the session will include brief presentations on the direction and enablers of the CoP and three practice innovations: point of care access to actionable evidence; family physician, consulting physician, and patient/caregiver communication; and innovations in engaging patients and caregivers. The other half will invite audience input into the direction of and offer opportunities for participation in the CoP and the proposed focus groups.
